REFERRERS
* All referrals are to be sent via email to: drjassessment@gmail.com
* Referrals must include a patient email address as our initial, detailed correspondence with referred patients is provided via email.
* For assessments for ADHD, we accept referrals for children aged between 6 to 16 years, 11 months.
* For assessments for ASD, we accept referrals for children aged between 2 to 16 years, 11 months.

PARENTS AND CARERS
* An appointment for your child will only have been made if your GP has submitted a referral, via email. If this has occurred, you will receive email confirmation from  drjassessment@gmail.com notifying you of the time and date of your child's appointment.

* If a referral for your child has not been submitted, please consult with your GP about doing so.

* All correspondence should be sent via email to: drjassessment@gmail.com

Email is the primary method of communication. If essential, telephone messages may be left on 0420 757 930. Please be aware, however, that there may be a delay in replying to phone messages as Dr J Paediatric Assessment operates only one day per week.
